Up to six bids could receive a share of between £2,000 and £4,000 each. Partnership bids will be looked on favourably, as will activities in rural areas.
Forms can be downloaded from: www.healthchallengedenbighshire.co.uk or you can receive a copy by phoning 01824 708370.
Completed forms should be sent to Sian Bennett at the Health, Social Care and Well-being Unit, Denbighshire County Council, 64 Brighton Road, Rhyl, LL18 3HP by September 25th.
All applicants will hear the outcome of their bid within three weeks of the closing date.
